Adult Questionnaires

13 (or more) questions. 

Forms need at least:

●      5 questions from the symptom domain

●      2 questions from the social domain

●      1 question from the functioning domain

●      1 question from the risk domain

●      1 question from the substance domain

●      3 questions about alliance

●      Additional questions can be selected from any domains you want, provided you can fit it on one page

Youth Questionnaires

18 (or more) questions.

Forms need at least:

●      5 questions from the symptom domain

●      2 questions from the social domain

●      1 question from the functioning domain

●      1 question from the risk domain

●      1 question from the substance domain

●      3 questions about alliance
