*Paper forms usage submitted via fax may require up to 24 hours to populate in Toolkit due to human processing data verification and is priced at an additional $1/form.

  • $30/month per active clinician/care coordinator for health plans with over 200 users
  • Services include:
    • Unlimited access to the Toolkit for clinicians/care coordinators, staff, care managers and administrators.
