Mobile Forms


Method 1: Creating a generic reusable form

Set up a tablet device that can easily be used to administer forms before the session.

Creating the form:

1)    Log into ACORN on the device you want to use.

2)    Cursor over the "Forms" Menu & select "Mobile Forms"

3)    This should take you to the page for creating your form

4)    Select the site this form will be used at (if you use sites)

5)    Select your clinician ID (or select ALL if you would prefer to select the clinician each time on the form itself )

6)    Select the form you want to use

7)    In the client ID field, enter N/A. This will let you fill out the client ID each time you use this form

8)    Send Form Via = Display URL

9)    Hit Go

10)  You've successfully generated your form! You will now receive a URL hyperlink to your form.

11)  Now you just need to save the URL Hyperlink you generated as a bookmark on your device! (Don't save the hyperlink to the page we used to generate the form)

The form you just created can be reused infinitely - you'll just need to change the client ID (and clinician ID if you set that to ALL) each time on the form itself before you give it to your client. See steps below.

 

Administering the form:

1) When the client arrives, click on the bookmark we created on the device you'll use to administer the form.

2) Enter the client ID (and select the clinician ID if you have the option here) and hand the form over to to the client to fill out.

  • When the client is done, take the tablet from them and hit “submit form”, if they haven't already.

3a) Once the form has been submitted, you can see the form image on the tablet itself.

OR

3b) To look at the form on your computer, navigate to the "Forms Processed" page (in your "Home" menu) in the toolkit. This will list all forms we have received from you. If they have submitted their form, it should show up here immediately. Find their submission and click on the "Form File" field to look how they filled out the form.

4) Be sure to look over their answers at each session for any unexpected indicators

5) Be sure to reset, or reload the form on the tablet so that it's blank for the next client.


Method 2: Creating unique forms specific to each client

    Set up QR codes that link to forms for each individual client. Useful for EMR integration.


Creating the form:

1)    Log into ACORN

2)    Cursor over the "Forms" Menu & select "Mobile Forms"

3)    This should take you to the page for creating your form

4)    Select the site this form will be used at (if you use sites)

5)    Select your clinician ID

6)    Select the form you want to use

7)    Enter the appropriate client ID

8)    Send Form Via = Display URL

9)    Hit Go

10)  You've successfully generated your form! You will now receive a QR Code to your form.

11)  You can print out the page with the QR code and keep it in a file with the client’s name on it, or you can save that image on your computer or even copy it into your client's file in your EMR. 

The form you just created can be reused infinitely with the client you set it up for. See steps below.

 

Administering the form:

1) When the client arrives, pull up the QR code and scan it with a QR reader app on your tablet

  •  This barcode is unique for each client

2)    Hand the tablet to the client and ask them to fill out the questionnaire

  •  When the client is done, take the tablet from them and hit “submit form”, if they haven't already.

3a) Once the form has been submitted, you can see the form image on the tablet itself.

OR

3b) To look at the form on your computer, navigate to the "Forms Processed" page (in your "Home" menu) in the toolkit. This will list all forms we have received from you. If they have submitted their form, it should show up here immediately. Find their submission and click on the "Form File" field to look how they filled out the form.

4) Be sure to look over their answers at each session for any unexpected indicators

5) Close out the form on the tablet when you're done.

 


Method 3: Texting the form to your client

     Create a form specific to your client, then text it to them. No app required.


Sending out the form:

1)    Log into ACORN

2)    Cursor over the "Forms" Menu & select "Mobile Forms"

3)    This should take you to the page for creating your form

4)    Select the site this form will be used at (if you use sites)

5)    Select your clinician ID

6)    Select the form you want to use

7)    Enter the appropriate client ID

8)    Send form via = Text Message (Change the drop-down to this)

9)    Enter client’s phone number

10) Message should populate automatically

11) Hit Go

Viewing their submission:

1) Once the client submits the form, you can immediately see the form image on your computer.

2) Navigate to the "Forms Processed" page (in your "Home" menu) in the toolkit.

3) This will list all forms we have received from you. If they have submitted their form, it should show up here immediately. Find their submission and click on the "Form File" field to look how they filled out the form.

4) Be sure to look over their answers at each session for any unexpected indicators
