What Type Of Form Should I Use?
Online Submission:
MOBILE FORMS
-Works on smartphone, table, or desktop.
-There is no ‘app’ to download.
-We recommend using inexpensive tablet devices to administer these forms

How Do I create one?
ADOBE ONLINE FORMS
-Must be used with Adobe, not your browser PDF Viewer.
-You can have the client fill out and submit these forms using Adobe Reader on a computer.
-How do I create one?
Paper Submission
-Print out paper forms to be filled out and faxed in. The fax number is listed at the bottom of the forms (800-961-1224.)
-Since these forms will need to be processed by our staff, they will take a bit longer. Expect a 24-48 hour delay for processing.
-Be sure to write cleanly and legibly to minimize errors.
